The familial aggregation of chronic pain disorders suggests a genetic contribution to this condition. In this study, we aimed to quantify the degree of genetic and environmental contribution to the inheritance of electrocortical activity to experimentally induced pain. Thirty healthy monozygotic and thirty-eight healthy dizygotic twins were exposed to electrical stimulation of the left forearm. Pain was rated as a subjective rating from 0 (no pain) to 100 (most severe pain). EEG was recorded from 60 scalp electrodes. Correlation analysis between twin pairs of pain ratings showed a significant heritable effect (0.21+/-0.02). Heritability estimates, obtained by univariate twin analysis, showed a significant contribution of genetic factors (0.34+/-0.05). In the mean, monozygotic twins were more similar to each other than dizygotic twins. The heritability estimates obtained by fitting individual pain ratings to a linear mixed model revealed a contribution of genetic factors of 0.41+/-0.04, meaning that a 41% of the variation of pain ratings could be attributed to the genetic influences. The genetic correlations between pairs of pain ratings were high (0.61+/-0.08). Similarity of pain ratings between dizygotic twins was smaller than between monozygotic twins, reflecting the impact of shared environmental factors in pain processing. Covariance analysis revealed that individual pain ratings shared more than 60% of the variance with electroencephalographic measures.
